Contracts

  • Educational Affiliation Agreement template is to be used for supervised experiences in clinical practice settings that take place between Rutgers School of Health Professions and an outside facility.
  • Educational Agreement MOU Short Form template is to be used when accepted by the site, for supervised experiences in clinical practice settings that take place between Rutgers School of Health Professions and an outside facility.
  • Template A Non-Clinical Grad Projects at students work site is to be used in or out of state when a student will perform non-clinical duties at their place of employment to complete their project requirement.
  • Template B Non-Clinical Grad Projects Graduate student project not at work site is to be used only for non-clinical duties by students taking place outside their place of employment in order to complete project requirements – UPDATED 06/2023!
  • Template C Clinical Grad Projects is to be used for clinical assignments that take place in or outside the state of NJ to satisfy project completion requirements.

Confidentiality and Non-Disclosure Agreement template is to be used any time you wish to document that you are exchanging confidential information or knowledge with another party and you wish to restrict access to or by third parties.
