Purpose

This policy establishes the SHP policy for requesting and approving changes to a student’s degree type, program, or concentration.

Accountability

The Dean of SHP, the Associate Dean for Academic Affairs, and appropriate Program Directors insure compliance with this policy. The SHP Manager of Admissions is responsible for implementing the Policy.

Policy

If a student wishes to request a change of his/her current program, concentration or degree type he/she must submit the Request to Change Program/Degree/Concentration form to the Office of Admissions. The student must submit all required documents and must satisfy the requirements of the new program that are in effect at the time of entry, e.g. GPA requirement, updated licensure, transcripts, and any other program requirements for the new academic area he/she is requesting to enter. There is no fee for this process.

Procedures

The Office of Admissions will collect all of the submitted documents and forward to the new area for a decision. Once the department has made their decision, they will return the completed form to Admissions, which will make the appropriate changes. Admissions will then advise the student of the decision, and if approved, will advise the student of any additional requirements (i.e. criminal background check, immunizations, the need to sign a new Requirement for Graduation form) as well as fulfill any other requirements that have been established.

Students who receive approval to change majors/concentrations are required to follow the Requirements for Graduation which is in effect at the time of the change of major. Previously earned credits for courses in which the student earned a passing grade (varies by program) within the past five years or the range for Satisfactory Academic Progress may be applied to the new major upon approval of the Program Director. At the discretion of the Program Director and the Dean, a minimum of 25% of the credits required for the new major must be earned following the change of major.
