Become a Patient
The H.O.P.E. Clinic provides FREE primary care to uninsured adults aged 18 and over. Residency not required.
To best care for all our patients, we are unable to accommodate walk in appointments. Please call to schedule an appointment on the day you would like to be seen.
Please call the office at 732-209-5638 on Monday or Wednesday beginning at 9:00 A.M. to request an appointment on the same day.
Our Services
We provide the following NO COST services:
- General health assessments
- Acute visits
- Chronic disease management (such as Diabetes and Hypertension)
- Referrals for free cancer screenings (such as mammograms, colon cancer screening, pap smears, prostate cancer)
- Health education and health system navigation
- Prescribe Medications as Indicated.
- Limited on-site laboratory testing (such as CMP, lipid panel, hemoglobin, urinalysis, pregnancy testing). We refer to an outside laboratory for other tests as needed
- Pre-employment assessments (as possible based on testing requirements)
When your care is beyond what we can provide or you need emergency or specialty care, we will refer you to outside locations..
Know Before You Go!
Our services are free and intended for those without health insurance.
- Please bring your medications to every visit.
- Documentation of residency is not required.
- We want you to understand your health. We have a language access plan that allows us to communicate with you in your preferred language.
- We work as a team. A Physician Assistant (PA) is a healthcare provider who works with a physician to care for your medical condition. We all work together to offer you the best access to care.
- Students are part of our team. A student will assist in all aspects of your care. Our students are in professional programs and always supervised appropriately.
- We take time to get to know you. We value each patient and ask many detailed questions. Sometimes we perform lab tests while you are here.
